To be eligible for Dial-a-Ride membership you must have a permanent or long-term disability which severly restricts your ability to use public transport services.

You are automatically eligible for membership of Dial-a-Ride if you are:

a current member of Taxicard
in receipt of Higher Rate Mobility Component of Disability Living Allowance
registered blind
in receipt of Higher Rate Attendance Allowance
in receipt of a War Pension Mobility Supplement

If you do not fulfill any of the criteria above, you may still be eligible for Dial-a-Ride but you may have to provide further evidence of your mobility problems.

How to apply

If you think you may be eligible for Dial-a-Ride membership:

Print this form, and complete it using the guidance notes
Once completed, post the form to:
